USAA holds down costs by
(a) eliminating services (call in and you go through menu hell, repeated interogatitons, multiple transfers with lengthy holds) and then if you are lucky someone who cannot help you or if unlucky a phone disconnect.
(b) tripling premiums for a minor claim even one in which you were not at fault. 37 years with them (car, home, umbrella) starting while on active service in USAF. One claim for a small fire while not at home, total payout under $3k after denying most damage, premium increases 3X
